UnitedHealth Group, via Star Hospice and LHC Group network, seeks an Afterhours Registered Nurse to deliver comprehensive hospice care after regular hours, including nights and weekends, with a focus on timely assessments and compassionate support for patients and families.

Responsibilities include triaging needs by phone, initiating in-person visits within 48 hours of referral, and conducting admission, death, and recertification visits.

Formación

• Current unrestricted RN licensure in the state of practice.
• Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
• CPR certification or ability to obtain within 90 days of hire.
• 1+ years of clinical experience.

Responsabilidades

• Triage patient needs by phone until a nurse can visit in person.
• Respond to after-hours crisis calls with urgency.
• Conduct nursing evaluation within 48 hours of referral.
• Perform after-hours visits including admissions, death, recertification, and routine visits.
• Assess and reassess patient/family needs at least every 15 days.
• Document problems, goals, interventions, and patient/family response.

Conocimientos
