Cape Fear Valley Hospice, part of LHC Group and connected to Optum, is hiring an Afterhours Registered Nurse for full-time hospice care. This role delivers comprehensive afterhours visits and admissions, triage by phone, and ensures timely evaluation within 48 hours of referral.

Requires NC RN licensure, hospice/home health experience, and ability to travel a 50-mile area. Night and weekend on-call shifts apply, with a rotating schedule.

Qualifications

• Active RN license in North Carolina required.
• Prior hospice and/or home health experience required.
• Must be able to travel throughout the agency's 50-mile service area.

Responsibilities

• Triage patient needs over the phone until an in-person visit is available.
• Respond to crisis calls after hours with urgency.
• Perform initial nursing evaluation within 48 hours of referral for eligibility of hospice services.
