Job summary

SouthernCare is seeking an Afterhours RN Care Manager to provide high-quality hospice care outside regular hours. The role emphasizes direct patient care during nights, weekends, and on-call periods, with strong clinical oversight for assigned patients.

The position involves coordinating with physicians, families, and the hospice team, maintaining documentation, and supporting quality outcomes. State nursing licensure and relevant hospice experience are required.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum one year of RN experience in acute care, oncology, long-term care, hospice, or home health.
• Must meet any state-specific requirements.

Responsibilities

• Provide direct in-person patient care during on-call, weekend, and after-hours shifts.
• Assess physical, emotional, psychosocial, and spiritual needs.
• Develop and revise the Hospice Plan of Care with physicians and IDG members.
• Educate patients and families on disease progression, symptoms, medications, and hospice philosophy.
• Maintain accurate, timely clinical documentation including assessments and care plans.
• Coordinate with the IDG to ensure continuity of care.
