Job summary

Heartland Hospice is seeking an Afterhours RN Care Manager to deliver high-quality hospice care outside standard business hours. You will provide direct patient care during nights, weekends, and on-call shifts, assess needs, and coordinate with the IDG for seamless care.

The role requires an active RN license in the state, CPR certification, and the ability to manage plans of care, documentation, and communications with physicians and families.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum one year of RN experience in acute care, oncology, long-term care, hospice, or home health.
• Must meet any state-specific requirements.
• Active RN license and CPR certification (in-person).
• Valid driver’s license, reliable transportation, and current auto insurance.

Responsibilities

• Provide direct in-person patient care during after-hours shifts, including assessments and visits.
• Coordinate with the IDG to ensure continuity of care.
• Develop and revise the Hospice Plan of Care with physicians and IDG members.
• Educate patients and families on disease progression and hospice philosophy.
• Maintain accurate clinical documentation and care plans.
