Job summary

SouthernCare seeks an Afterhours RN Care Manager to provide compassionate hospice care during nights, weekends, and on-call hours. You will assess patients, coordinate with the IDG, and ensure continuity of care with physicians and families.

Responsibilities include direct patient visits, admission workups, care plan updates, and thorough documentation. A current Alabama RN license and hospice experience are required; strong communication and leadership are essential.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum one year of RN experience in acute care, oncology, long-term care, hospice, or home health.
• Must meet any state-specific requirements.

Responsibilities

• Provide direct in-person patient care during after-hours shifts, including assessments and visits.
• Coordinate with IDG, physicians, and families for ongoing plans of care.
• Develop and revise Hospice Plan of Care in collaboration with care team.
• Maintain accurate clinical documentation and ensure timely updates.
