Job summary

SouthernCare New Beacon Hospice is seeking an Afterhours RN Care Manager to provide high-quality hospice care outside standard business hours, ensuring seamless coverage and strong clinical oversight for patients and families during nights, weekends, and on-call hours.

The role includes direct patient care, comprehensive assessments, and active participation in the Interdisciplinary Group (IDG) to coordinate care with physicians and staff.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum one year of RN experience in acute care, oncology, long-term care, hospice, or home health.
• Must meet any state-specific requirements.

Responsibilities

• Provide direct in-person care during after-hours shifts including assessments and admissions.
• Conduct initial and ongoing assessments of physical, emotional, psychosocial, and spiritual needs.
• Educate patients and families on disease progression and hospice care.
• Maintain accurate clinical documentation and care plans.
• Support IDG processes with chart reviews and interdisciplinary communication.
• Ensure infection control and safety practices.
