Job summary

Gentiva’s SouthernCare New Beacon Hospice is seeking a dedicated Afterhours RN Care Manager to provide high-quality hospice care outside standard hours. You will deliver compassionate, patient-centered care on nights, weekends, and on-call hours, ensuring seamless coverage and continuity of care.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum one year of RN experience in acute care, oncology, long-term care, hospice, or home health.
• CPR certification (in-person) and valid driver’s license.

Responsibilities

• Provide direct in-person care during on-call and after-hours shifts.
• Conduct initial and ongoing assessments of patients and families.
• Coordinate with IDG and update the plan of care as needed.
• Maintain accurate clinical documentation and notes.
