Job summary

Gentiva Hospice seeks an Afterhours Support RN to deliver compassionate hospice care outside standard hours, covering nights, weekends, and on-call periods. You will provide direct patient care, perform assessments, and coordinate with the Interdisciplinary Group to ensure seamless care.

The role requires RN licensure in the state of employment, at least one year in acute care, oncology, long-term care, hospice, or home health, and strong communication and leadership skills to support field

Qualifications

Strong knowledge of hospice principles and regulations.
Ability to conduct thorough patient assessments and develop care plans.
Effective communication with patients, families and interdisciplinary team.
Demonstrated leadership to support field staff.
Proficiency with EMR systems and Microsoft Office.

Responsibilities

Provide direct in-person patient care during after-hours shifts including assessments and visits.
Maintain primary clinical oversight and coordinate with IDG members.
Update and revise the Hospice Plan of Care with physicians and IDG input.
Educate patients and families on disease progression and symptom management.
Ensure accurate documentation and compliance with regulatory standards.
