Job summary

SouthernCare New Beacon Hospice seeks an Afterhours RN Care Manager to provide high-quality hospice care outside standard hours in Alabama. You will perform after-hours assessments, admissions, routine visits, and live discharges while maintaining clinical oversight and coordinating with the IDG.

The role requires active RN license in Alabama, at least one year in hospice or related field, strong communication, and proficiency with EMR systems.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum one year of RN experience in acute care, oncology, long-term care, hospice, or home health.

Responsibilities

• Provide direct in-person care during after-hours shifts, including assessments and admissions.
• Maintain primary clinical oversight for assigned patients.
• Collaborate with the IDG and physicians to update plans of care.
• Document assessments, care plans, and notes accurately.
• Support scheduling and triage for visit requests.
