Job summary

Elite Hospice, part of LHC Group and the Optum family, is seeking a Per Diem After Hours Registered Nurse to deliver comprehensive hospice care in Round Rock, TX, after regular business hours, including nights and weekends. You will triage patient needs by phone, respond to crises with urgency, and complete initial nursing evaluations within 48 hours of referral, followed by after-hours visits as needed.

Documentation and family support are essential.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Current CPR certification.
• Minimum one year of clinical experience.
• Current driver's license with vehicle insurance and access to a dependable vehicle or public transportation.

Responsibilities

• Triage patient needs over the phone until an in-person nurse visit.
• Respond to crisis calls after-hours with urgency.
• Conduct initial nursing evaluation to determine hospice eligibility within 48 hours of referral when assigned.
• Perform after-hours visits including admissions, death, recertification, and routine visits.
• Identify patient/family needs and reassess at least every 15 days.
• Document problems, goals, interventions, and patient/family response.
