Agape Care Group in Virginia is seeking compassionate After-Hours Hospice Registered Nurses to cover on-call from 5pm Friday to 8am Monday. You will work with our 24/7 Triage Department and local hospice team to deliver high‑quality, patient‑centered care after hours.

As a patient advocate, you’ll coordinate with patients, families and the care team to ensure the plan of care is carried out, while enjoying leadership support and ongoing professional development opportunities.

Qualifications

• Education: Graduate of an accredited school of nursing with a current state license as a registered nurse.
• 2+ years of nursing experience in a clinical care setting (hospice experience preferred).
• Reliable transportation and ability to lift 80-100 lbs as needed.

Responsabilités

• Serve as the patient advocate and ensure the plan of care is followed after hours.
• Coordinate communication among patient, family, and hospice care team.
