Job summary

Gentiva is seeking a Registered Nurse (RN) for Hospice, delivering compassionate after-hours care in Petersburg, VA. The role focuses on responding to patient needs during evenings, nights, weekends, and holidays, with a 7 on/7 off schedule and coordination with the daytime team.

You will provide direct patient care, document visits, and collaborate with an interdisciplinary hospice team to guide care plans and symptom management, supporting families in stressful times.

Qualifications

• Current RN license in the state of requested employment (or eligibility to obtain).
• 1+ years of nursing experience.
• Hospice heart: compassionate, calm and dependable in urgent care situations.
• Excellent clinical judgment and ability to provide support in intense settings.
• Comfortable providing care in patients' homes or wherever they reside.
• Valid driver’s license and reliable transportation.
• Current CPR certification.

Responsibilities

• Report directly to the Administrator, Executive Director, or Patient Care Manager Senior.
• Provide direct patient care and support after normal business hours.
• Serve as the primary liaison between patients, families, physicians, and the hospice team during your shift.
• Respond promptly and compassionately to urgent calls, patient visits, and symptom management needs.
• Evaluate patient and family needs to guide clinical decisions and implement the plan of care.
• Support and collaborate with an interdisciplinary group including LPNs/LVNs, CNAs, and other staff.
• Accurately document clinical visits and communications.
• Participate in quality assessment and performance improvement programs.
• Work independently while ensuring continuity of care with daytime team.
