Twin Cities Physicians in Minnesota is seeking a full-time Mobile Wound Care Nurse Practitioner to provide advanced wound care across senior living communities in the Twin Cities Metro. You'll care for residents with complex medical needs, coordinating with physicians and care teams to improve healing, quality of life, and reduce hospitalizations.

Requires Minnesota CNP license (AGNP preferred), travel across the metro, and a commitment to compassionate, patient-centered care.

Qualifikationen

• Active Minnesota CNP license (AGNP preferred).
• Experience in wound care, geriatrics, or post-acute settings.
• Strong clinical judgment and ability to manage complex patients.
• Valid driver’s license and willingness to travel across Twin Cities Metro.

Aufgaben

• Provide advanced wound care assessments, treatment planning, and ongoing management of complex wounds.
• Conduct rounding visits in assisted living, memory care, skilled nursing, and long-term care communities.
• Manage medications, diagnostics, treatment plans, and follow-up care.
• Collaborate with physicians, nurses, therapists, caregivers, and community staff.
• Support proactive interventions that improve healing outcomes and reduce avoidable complications.
• Help patients maintain the highest possible quality of life throughout their healthcare journey.
