Theoria Medical is seeking a full-time Advanced Practice Provider to serve local skilled nursing facilities, delivering high-quality, patient-centered care within a mission-driven team.

Qualifications

• Active U.S. license and board certification in applicable APRN/PA field.
• Ability to provide high-quality, patient-centered care in SNF settings.
• Experience with chronic care management and interdisciplinary teams.

Responsibilities

• Provide high-quality, patient-centered care to residents in skilled nursing facilities.
• Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues.
• Communicate with facility nursing staff via phone or secure messaging as needed.
• Maintain reliable on-site attendance for scheduled shifts.
• Ensure compliance with clinical standards, facility policies, and regulatory requirements.
