Job summary

Theoria Medical is seeking an Advanced Practice Provider to support local skilled nursing facilities. You will deliver high-quality, patient-centered care, perform rounds, and manage treatment plans in a flexible and autonomous role.

The provider will operate within a collaborative clinical team, using intuitive technology to streamline charting and ensure continuity of care for vulnerable populations.

Qualifications

• AP providers must hold an active U.S. license and be board-certified by ANCC, AANPCB, PNCB, or AACN (for NP) or NCCPA (for PA).
• Specialties include Internal/ Family/ Emergency/ General Practice, PM&R, and Geriatrics.

Responsibilities

• Provide high-quality, patient-centered care in skilled nursing facilities.
• Conduct daily rounds and develop treatment plans for residents.
• Communicate with facility staff via phone or secure messaging as needed.
