Theoria Medical is seeking a full-time Advanced Practice Provider to serve local skilled nursing facilities within a mission-driven care community. Qualifications

• Active U.S. license and board certification by ANCC, AANPCB, PNCB, or AACN for NPs, or NCCPA for PAs.
• Specialties include internal medicine, family medicine, emergency medicine, general practice, physical medicine and rehabilitation, and geriatric medicine.

Responsibilities

• Provide high-quality, patient-centered care to residents in skilled nursing facilities.
• Conduct daily rounds, prescribe medications and treatment plans, and address acute/emergent issues.
• Communicate with facility nursing staff via phone or secure messaging as needed.
• Maintain reliable on-site attendance for scheduled shifts.
• Ensure compliance with clinical standards, facility policies, and regulatory requirements.
