Job summary

Theoria Medical is seeking a full-time Advanced Practice Provider to serve local skilled nursing facilities, offering flexible scheduling and autonomy within a collaborative clinical team.

You will conduct daily rounds, prescribe medications, and coordinate care for residents, supported by an intuitive EHR, UpToDate access, and an AI transcription tool to streamline documentation. Candidates must hold an active U.S. license and be board-certified in one of the recognized AP/PA boards.

Qualifications

• Active U.S. license required.
• Board certification by ANCC, AANPCB, PNCB, AACN (APN) or NCCPA (PA) required.

Responsibilities

• Provide high-quality, resident-centered care in skilled nursing facilities.
• Conduct daily rounds and develop/adjust treatment plans.
• Prescribe medications and coordinate with facility staff.
• Ensure timely charting and regulatory compliance.
