Theoria Medical is seeking a full-time Advanced Practice Provider to care for residents in local skilled nursing facilities. You will deliver patient-centered care, conduct daily rounds, and develop treatment plans as part of a collaborative clinical team.

Qualifications

• Active U.S. medical license and board certification as applicable (NP or PA).

Responsibilities

• Conduct daily rounding and manage meds/treatment plans.
