Job summary

Intermountain Health in Wheat Ridge, CO is seeking an experienced Nurse Practitioner or Physician Assistant to join our Heart Failure Clinic.

You will partner with a certified Heart Failure Cardiologist to manage stage 3 and 4 heart failure patients, ensure medication adherence, monitor diet and lifestyle changes, and participate in both outpatient visits and inpatient rounding to coordinate care across the hospital and clinic.

Qualifications

• Licensed as NP or PA in CO, or able to obtain license.
• DEA and full prescriptive authority in CO, or able to obtain.
• BLS and ACLS certification required.
• Minimum 2 years of NP/PA experience in a Heart Failure/Cardiology setting.

Responsibilities

• Provide care for stage 3 and 4 heart failure patients.
• Coordinate continuity of care from hospital to clinic and with primary care physicians.
• Medication management and monitoring dietary and lifestyle changes.
• Outpatient consults and follow-ups with inpatient rounding.

Job Description: Advanced Practice Provider for Cardiology Heart Failure Program - Wheat Ridge, CO

About this role

Intermountain Health is seeking an experienced Nurse Practitioner or Physician Assistant for our Heart Failure Clinic. The selected candidate will partner with the a certified Heart Failure Cardiologist to continue to build a pre-eminent Heart Failure Program in the state of Colorado. You would practice at our Wheat Ridge Cardiology clinic and Lutheran Hospital. Additional locations may be added with program growth.

Hours are 4/10's Mon-Fri, no call and no weekends. Your day off will vary.

You would work with stage 3 and 4 heart failure patients to ensure medication compliance, monitoring dietary and lifestyle changes, regular follow ups and much more.

This position would be part of the continuity of patient care from hospital to clinic and back to the primary care physician or cardiology service line.

You would work a combination of outpatient clinic consults and follow ups along with inpatient rounding and consults.
