Job summary

Intermountain Health is seeking an experienced Nurse Practitioner or Physician Assistant to join the Heart Failure Clinic at Wheat Ridge, CO. You will partner with a certified Heart Failure Cardiologist to enhance a premier program in Colorado.

Role includes outpatient consults, follow-up visits, and inpatient rounding, with 4/10 workdays, no calls, and no weekend shifts. Competitive compensation and benefits are offered in a supportive team environment.

• DEA and full prescriptive authority in CO or able to obtain.

Responsibilities

• Provide outpatient clinic consults and follow-ups for heart failure patients.
• Inpatient rounding and consults as part of hospital-to-clinic continuity of care.
• Collaborate with a Heart Failure Cardiologist to manage medications, monitoring, and lifestyle guidance.

Advanced Practice Provider for Cardiology Heart Failure Program – Wheat Ridge, CO

Intermountain Health is seeking an experienced Nurse Practitioner or Physician Assistant for our Heart Failure Clinic. The selected candidate will partner with a certified Heart Failure Cardiologist to continue to build a pre‑eminent Heart Failure Program in the state of Colorado.

About This Role

• You will practice at our Wheat Ridge Cardiology clinic and Lutheran Hospital. Additional locations may be added with program growth.
• Hours are 4/10’s Mon-Fri, no call and no weekends. Your day off will vary.
• You will work with stage 3 and 4 heart failure patients to ensure medication compliance, monitoring dietary and lifestyle changes, regular follow ups and much more.
• This position will be part of the continuity of patient care from hospital to clinic and back to the primary care physician or cardiology service line.
• You will work a combination of outpatient clinic consults and follow ups along with inpatient rounding and consults.

Hours & Compensation

40 hours per week. Hourly rate: $57.33 – $88.47 (actual rate depends on experience).
