Responsibilities
- Provide exceptional primary care and conduct office visits for routine and acute issues.
- Administer annual wellness visits and health risk assessments with accurate documentation.
- Coordinate care with other providers, specialists, and testing facilities.
- Lead population health efforts in coordination with the Care Team.
- Educate patients on health conditions, care plans, and treatments.
- Conduct home visits as needed and participate in promotional activities.
