Job summary
Oak Street Health in Aurora, CO is seeking a Nurse Practitioner to provide value-based primary care to adults on Medicare. You will work with a collaborative care team to deliver holistic care, health risk assessments, and care coordination. Strong NP certification and active licenses required.

Qualifications
- Master of Science in Nursing or higher for NP practice.
- State NP license active and in good standing.
- National certification in recognized NP specialties.
- DEA license for prescription authority.
- US work authorization to work in the United States.
Responsibilities
- Provide exceptional primary care to adults on Medicare.
- Conduct office visits for routine and acute issues.
- Administer annual wellness visits and health risk assessments.
- Coordinate care with other providers and facilities.
- Lead population health initiatives with the care team.
- Educate patients on health conditions and treatment plans.
- Participate in promotional activities and community outreach.
- Conduct home visits as needed.
