Qualifications

• MSN/DNP or NP certificate required.

Responsibilities

• Provide effective primary care to adults on Medicare.
• Conduct office visits for routine and acute issues.
• Administer wellness visits and health risk assessments with detailed documentation.
• Coordinate care with physicians, specialists, and other care team members.
• Lead population health initiatives and participate in outreach.
• Educate patients about conditions, plans, and treatments.
• Engage in promotional activities and home visits as needed.
• Collaborate in a multidisciplinary outpatient clinic setting.
