Job summary

Clare Medical of New Jersey is seeking a Nurse Practitioner to perform comprehensive patient assessments, wellness and sick visits, and manage chronic diseases. You will order and interpret tests, collaborate with other providers, and prescribe therapies in a collaborative practice.

Applicants should hold an unrestricted NJ APN license with at least 1 year of clinical experience and a current DEA/CDS license. Travel within the region is required; bi-lingual skills are a plus.

Qualifications

• Unrestricted APN license in NJ with at least 1 year clinical experience.
• Current DEA/CDS license.
• Willingness and ability to work independently.
• Bi-lingual a plus.

• Wellness exams and sick visits
• Evaluate and manage common and acute chronic diseases
• Order, interpret and evaluate diagnostic and lab results
• Refer and collaborate with other providers
• Prescribing pharmacological and non-pharmacological therapies

REQUIREMENTS:

• Computer literate & EMR Proficiency; Experience with Athena a plus
• Unrestricted APN license in the State of NJ with at least 1 year clinical experience
• Current DEA/CDS
• Willingness and ability to work independently
• Personable, passionate and energetic
• Ability to travel within the region
• Previous experience with Medicare eligible patients a plus.
• Bi-lingual a plus
