Job summary

Clare Medical of New Jersey is seeking an Advanced Nurse Practitioner to join our visiting physician practice serving Ocean County. You will perform in-home assessments, wellness exams, and sick visits, and you will evaluate and manage common and chronic diseases in patients at home.

The position offers a sign-on bonus, travel reimbursement, flexible scheduling, and strong clinical collaboration with Medical Director support.

Qualifications

• Unrestricted APN license in NJ with at least 1 year clinical experience.
• Current DEA/CDS license.
• Willingness and ability to work independently.
• Personable, passionate and energetic.
• Ability to travel within the region.
• Experience with Medicare eligible patients a plus.

REQUIREMENTS:

• Computer literate & EMR Proficiency
• Unrestricted APN license in the State of NJ with at least 1 year clinical experience
• Current DEA/CDS
• Willingness and ability to work independently
• Personable, passionate and energetic
• Ability to travel within the region
• Previous experience with Medicare eligible patients a plus.
