Job summary

RWJBarnabas Health is seeking a Full-Time Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join the Primary Care APP Float Pool (South) in southern New Jersey. This onsite role includes regional coverage with periodic telemedicine support and a Monday–Friday day shift.

Qualifications include a Master's degree or equivalent, active NJ APN or PA licensure, BLS, and NP/PA certifications along with DEA and NCCPA as applicable.

Qualifications

• Master's degree or graduate level certification required.
• Current NJ APN licensure.
• Nurse Practitioner Certification and RN License required.
• PA license and NCCPA certification required for physician assistant pathway.

Responsibilities

• Perform comprehensive initial and episodic health assessments.
• Collaborate with interdisciplinary team to develop and implement care plans.
• Initiate diagnostic, therapeutic and nursing actions with physician oversight.
• Provide follow-up care including acute and routine examinations.
• Monitor medications per protocol and adjust as needed.
• Coordinate referrals and triage patient issues as necessary.
• Educate patients and families on health management and prevention.

Essential Functions

• Performs comprehensive initial and episodic health assessments to identify existing or potential health problems
• Initiates and implements an individualized nursing and medical plan of care in collaboration with members of the interdisciplinary team
• Initiates appropriate diagnostic, therapeutic, medical and nursing actions in conjunction with physician
• Provides follow-up care including acute and routine examinations
• Assesses progress, identifies problems, initiates preventive measures, recognizes emergent problems, initiates proper care in collaboration with physician
• Monitors and adjusts patients medication according to protocol or medical plan of care
• Implements nutritional therapies, as appropriate
• Triages patient problem calls which require medical attention
• Orders appropriate laboratory and diagnostic procedures
• Recognizes normal and abnormal laboratory findings and the pathology involved
• Documents information in patients charts following documentation standard
• Provides health education to the patient and family (e.g., management of side effects, anticipatory teaching)
• Manages medical emergencies as indicated, and according to standing orders
• Provides monitoring and continuity of care between health care settings
• Provides patient and family counseling and reassurance in regard to psychological and or psychosocial issues
• Initiates proper referrals to private or community agencies
• Acts as a resource and teacher for other members of the health care team
• Shares knowledge and expertise with staff
• Collaborates with the nursing and medical staff at partner and affiliate sites to coordinate continuity of care
• Evaluates the quality of care as well as the quality of life for patients through assessment of patient response and recommends interventions, as indicated

Travel Requirement

• Employee will be expected to provide both on-site and telemedicine support and coverage for practices as needed
• Provides coverage for all practice sites within the assigned regional territory
• Mileage reimbursement will be provided for travel to assigned locations from primary department
• Frequency of travel will be contingent on the practice need for the assigned location
• Duration of coverage will be contingent on the practice need for the assigned location
