Job summary

Clare Medical of New Jersey is seeking an APN/PA to perform housecalls across Northern Mercer and Somerset County, NJ. You will conduct detailed patient assessments, physical exams, wellness and sick visits, and manage common and chronic diseases.

The role requires an unrestricted NJ APN license, current DEA/CDS, and the ability to travel within the region. Excellent salary with bonus opportunities and travel reimbursement, plus a supportive work environment, await the right candidate.

Qualifications

• Unrestricted APN license in NJ with at least 1 year clinical experience.
• Current DEA/CDS license.
• Willingness and ability to travel within the region.
• Bi-lingual a plus.
• Computer literate and EMR proficiency.

PRIMARY RESPONSIBILITIES:

Initiate and complete detailed patient assessment & physical exams

Wellness exams and sick visits

Evaluate and manage common and acute chronic diseases

Order, interpret and evaluate diagnostic and lab results

Refer and collaborate with other providers

Prescribing pharmacological and non-pharmacological therapies

REQUIREMENTS:

Computer literate & EMR Proficiency

Unrestricted APN license in the State of NJ with at least 1 year clinical experience

Current DEA/CDS

Willingness and ability to work independently

Personable, passionate and energetic

Ability to travel within the region

Previous experience with Medicare eligible patients a plus.

Bi-lingual a plus
