Job summary

SOUTH PLAINS LEGACY OF LOVE HOSPICE FOUNDATION INC is seeking a Nurse Practitioner to deliver comprehensive hospice care under physician supervision. You will assess, diagnose, and manage terminal illnesses, directing a patient-centered plan of care in home and community settings.

Qualifications

• Graduation from an accredited School of Nursing and Nurse Practitioner Program.
• Current state NP license and CPR certification.
• Minimum 1 year NP hospice or palliative care experience.
• Certification in Hospice and Palliative Nursing (CHPN) preferred.
• Strong communication and documentation skills.

Responsibilities

• Provide medical care and symptom management for terminally ill patients under physician supervision.
• Perform comprehensive history and physicals and document findings.
• Develop individualized plans of care and coordinate with interdisciplinary team.
• Conduct hospice face-to-face visits and determine eligibility with the medical director.
• Educate patients and families and ensure continuity of care.
