Job summary

Heartland Hospice is seeking a Nurse Practitioner to deliver advanced, patient‑centered care across hospice and palliative services. You will perform comprehensive patient assessments, manage visits including Hospice F2F for certification, and collaborate with IDG/IDT to develop individualized care plans.

You will prescribe medications within state and regulatory scopes, coordinate care across settings, and maintain compliant documentation while supporting families through serious illness.

Qualifications

• Master of Science in Nursing (MSN) or higher from an accredited program.
• Completion of an accredited Nurse Practitioner program.
• Minimum of two years of Nurse Practitioner experience preferred.
• Experience in hospice, palliative care, oncology, geriatrics, family/adult medicine, home health, or acute care preferred.
• Experience with Part B billing preferred.

Responsibilities

• Complete hospice Face‑to‑Face visits within CMS timeframes.
• Collaborate with IDG/IDT and PCP to coordinate Plan of Care.
• Perform in‑depth patient assessments across physical, emotional, functional and psychosocial domains.
• Prescribe medications and treatments within regulatory parameters.
• Document findings accurately and ensure compliant medical records.
