Job summary

Shalom Hospice is seeking a Nurse Practitioner to deliver comprehensive hospice care under the supervision of a collaborating physician. You will assess patients, develop care plans, prescribe medications, and coordinate with families and the hospice interdisciplinary team to meet patient goals.

Responsibilities include performing history and physical exams, determining hospice eligibility, ensuring symptom control, documenting clinical notes, and educating patients and caregivers.

Qualifications

• Graduation from an accredited School of Nursing and NP program.
• Current DEA license, if applicable by state regulations.
• Current nursing licensure in state and CPR certification.
• Master’s degree with 1+ year NP experience and 1+ year hospice/palliative care experience.
• Certification in advanced practice nursing preferred (e.g., CHPN).

Responsibilities

• Provide medical care and manage terminal illness under physician supervision.
• Perform history and physical examinations and interpret tests.
• Develop individualized care plans and coordinate hospice services.
• Assess hospice eligibility and contribute to recertification discussions.
• Document clinical notes and orders accurately.
• Educate patients and families on care plans and end-of-life issues.
• Collaborate with the hospice interdisciplinary team to meet patient goals.
• Maintain safety and quality standards in care delivery.
