Qualifications

• Licensed registered nurse in North Carolina with current credentials.
• Experience in hospice or palliative care is preferred.
• Ability to perform face-to-face eligibility assessments and recertifications.

Responsibilities

• Provide advanced symptom management and clinical assessment for hospice patients.
• Collaborate with Hospice Medical Director, physicians, and IDG for care planning.
• Conduct Face-to-Face encounters for eligibility determinations and recertifications.
• Educate patients, families, and clinical staff on hospice care and medications.
• Participate in after-hours on-call coverage and regulatory readiness activities.
