Job summary

ADP, Inc. seeks a Nurse Practitioner to provide Hospice care to terminally ill patients at home and in clinical settings. You will support families, collaborate with the Hospice team, and participate in IDG planning.

Applicants must hold NP certification, be licensed to practice in the state, and demonstrate independent judgment, crisis response, and strong interpersonal skills. Team-oriented care delivery is a key focus.

Qualifications

• Graduate from an accredited school of nursing with NP certification.
• Current state NP license to practice as required.
• National board certification in Nurse Practitioner.

Responsibilities

• Regularly assesses Hospice patient needs and updates care plans.
• Orders, performs, supervises and interprets diagnostic tests and labs.
• Initiates and manages treatment plans, including prescribing meds.
• Educates and counsels patients and families about care at home.
• Maintains records and participates in team conferences with IDG.
• Coordinates interdisciplinary care for patients in SNF/NF/ICF settings.
• On-call coverage and responds to urgent patient needs as required.
