Job summary
Southcoast Health System is looking for an Advanced Practice Practitioner to join their Health at Home - Supportive Care Hospice Team. This full-time role involves delivering comprehensive hospice care to patients and families, collaborating closely with a Physician and other team members.
The successful candidate will have a Master's Degree in Nursing, a Nurse Practitioner certification, and at least five years of relevant nursing experience.
Qualifications
- Requires a Master's Degree in Nursing.
- Certification as a Nurse Practitioner is required.
- Current RN license in Massachusetts & Rhode Island required.
- 5+ years nursing experience in oncology, hospice, geriatrics, or ICU preferred.
- At least 1-year experience as a Nurse Practitioner.
- Certification in Hospice and Palliative Care required.
- Must possess current CPR certification.
Responsibilities
- Deliver hospice care and related health care services.
- Evaluate patients and families bedside.
- Assist with diagnosis, treatment, and management of health conditions.
- Educate patients and families on preventative care and medical problems.
