HarmonyCares is seeking an experienced Advanced Practice Provider to deliver in-home geriatric primary care. The role focuses on patient-centered care, managing chronic conditions in patients' homes Monday through Friday, without weekends or evenings.

The company offers a competitive base salary, 401K with match, CME/license stipends, UpToDate access, malpractice tail coverage, and minimal call duties. This is a physician-driven, team-based home health model across 14 states.

Qualifications

• Geriatrics/Home health experience preferred.
• Active clinical license in the state of practice.
• Board certification and DEA eligibility.
• CPR/BLS certification.
• Valid driver's license with good driving record.

Responsibilities

• Provide high-quality geriatric home health care to a patient panel.
• Manage chronic conditions within patients' homes Monday–Friday.
• Collaborate with a physician-driven team to coordinate care and address patient needs.

Responsibilities

The Advance Practice Providers we seek are those who have directly related experience in home health, geriatrics, hospice and primary care. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.

Qualifications

• Licensed as a NP or PA in the State where HarmonyCares Medical Group office located
• Federal DEA License/eligibility
• Geriatric training/experience preferred
• Active CPR/BLS Certification
• Board CertificationMust maintain a valid driver’s license and good driving record
