HarmonyCares is seeking a Nurse Practitioner to join our home-based primary care team serving New York patients. This role focuses on delivering patient-centered care to high-acuity individuals in the home setting, Monday through Friday. You will practice medicine with time to address patient needs, supported by a seasoned care team in a value-based model. A NY NP license, DEA, CPR/BLS, board certification, and a valid driver's license are required.

Competenze

• Licensed NP in the state where HarmonyCares Medical Group office is located.
• Federal DEA license/eligibility.
• Geriatric training/experience preferred.
• Active CPR/BLS Certification.
• Board Certification.
• Valid and unencumbered driver’s license with good driving record.

Mansioni

Responsibilities

The Nurse Practitioners we seek are those who have directly related experience in home health, geriatrics, hospice and primary care. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.
