HarmonyCares in Michigan is seeking an in-home value-based Advance Practice Provider to manage a high-risk geriatric panel. The role runs Monday through Friday with no weekends or evenings. You will use Athena EMR and Marvix AI Scribe to complete charting during the workday and support a physician-led, team-based model. Competitive benefits and malpractice coverage are included.

Competenze

• Licensed NP or PA in the state HarmonyCares Medical Group operates
• Active Federal DEA license or eligibility
• Geriatric training/experience preferred
• CPR/BLS Certification (current)
• Board Certification
• Valid and unencumbered driver's license with good driving record

Mansioni

• Provide home-based primary care to high-acuity geriatric patients, Mon–Fri
• Use Athena EMR + Marvix AI Scribe for charting during the workday

Responsibilities

The Advance Practice Providers we seek are those who have directly related experience in home health, geriatrics, hospice and primary care. The APP who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.

Qualifications

• Licensed as a NP or PA in the State where HarmonyCares Medical Group office located
• Federal DEA License/eligibility
• Geriatric training/experience preferred
• Active CPR/BLS Certification
• Board Certification
• Valid and unencumbered driver's license with good driving record
