Job summary

HarmonyCares is seeking a Nurse Practitioner to deliver in-home primary care for high-acuity patients across select markets. The role emphasizes patient-centered care over volume; you’ll address patients' needs with a comprehensive, team-based approach.

A state NP license, DEA eligibility, and geriatric background are required. Posted pay range is USD 145,000 to 160,000 per year, with a full benefits package, including 401K with match, CME/dues stipends, and tail malpractice coverage.

Qualifications

• Licensed as a NP in the state where HarmonyCares Medical Group office located.
• Federal DEA License/eligibility.
• Geriatric training/experience preferred.
• Active CPR/BLS Certification.
• Board Certification.
• Must maintain a valid driver’s license and good driving record.

Responsibilities

• Provide primary care to high acuity patients in their home.
• Deliver patient-centered care with focus on addressing needs, not volume.
• Work with an integrated team to coordinate care and support.

The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.
