Job summary

HarmonyCares in Michigan is seeking an experienced Advance Practice Provider to manage a high-risk geriatric, in-home patient panel. Workdays run Monday through Friday from about 8:00 AM to 4:30 PM, with no weekends or evenings, and charting supported during the day using Athena EMR plus Marvix AI Scribe.

The role emphasizes patient-centered care over volume, with a focus on coordinating care for complex cases in the home setting.

Qualifications

• Licensed NP or PA in the state where HarmonyCares Medical Group office is located.
• Federal DEA license/eligibility.
• Geriatric training/experience preferred.
• Active CPR/BLS Certification.
• Board Certification.
• Valid and unencumbered driver’s license with good driving record.

Responsibilities

• Provide primary care at patients' homes Monday–Friday, focusing on patient needs and quality of care.
• Deliver care with a patient-centered approach rather than volume.

The Advance Practice Providers we seek are those who have directly related experience in home health, geriatrics, hospice and primary care. The APP who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.
