HarmonyCares is seeking an Advance Practice Provider (NP/PA) to manage chronic conditions for high-risk geriatric patients in home settings. The role emphasizes patient-centered care with a focus on quality outcomes within a physician-driven, team-based model.

Ideal candidates hold NP or PA licensure in the state, with DEA eligibility, CPR/BLS certification, and geriatric experience. Competitive benefits accompany a base salary and CME support.

• Practice medicine focused on patient care over volume.
• Take time to address patient needs and coordinate with the care team.

Responsibilities

The Advance Practice Providers we seek are those who have directly related experience in home health, geriatrics, hospice and primary care. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.
