Job summary

HarmonyCares is seeking a Nurse Practitioner to provide in-home primary care for high acuity patients in our home-health program. The role emphasizes patient-centered care, thorough assessment, and collaboration with a team-based care model to improve outcomes at patients' homes.

The position requires NP licensure in the relevant state, DEA eligibility, and CPR/BLS certification, with preference for geriatrics experience and a valid driver's license for community visits.

Qualifications

• Licensed as a NP in the State where HarmonyCares Medical Group office located.
• Federal DEA License/eligibility
• Geriatric training/experience preferred
• Active CPR/BLS Certification; Board Certification
• Valid and unencumbered driver's license with good driving record

Responsibilities

• Provide direct home health care to high acuity patients in home settings.
• Address patient needs with a patient-centered approach, taking time as required.
• Work within an integrated, team-based care model to deliver primary care at home.

The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.
