HarmonyCares is seeking a Nurse Practitioner or Physician Assistant to deliver in-home primary care for a high-acuity geriatric patient panel in a value-based team setting. This Monday–Friday role avoids weekends and evenings, with dedicated support to manage chronic conditions.
You will collaborate with an interdisciplinary team and focus on patient-centered, quality care in the home. Qualifications include state NP/PA licensure, DEA, CPR/BLS, board certification, and a valid driver's license;

The Advance Practice Providers (NP/PA) we seek are those who have directly related experience in home health, geriatrics, and primary care. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.
