Job summary

HarmonyCares is seeking a Nurse Practitioner to deliver in-home geriatric primary care in Michigan, with a Monday–Friday schedule and no weekends or evenings. You will manage chronic conditions for your own patient panel and complete charting during work hours using Athena EMR with AI Scribe.

The role emphasizes patient-centered care, minimal call responsibilities, and a team-based model. A competitive base salary plus extensive benefits is offered.

Qualifications

• Licensed as a NP in the state where HarmonyCares Medical Group office located.
• Federal DEA License/eligibility.
• Geriatric training/experience preferred.
• Active CPR/BLS Certification.
• Board Certification.
• Must maintain a valid driver’s license and good driving record.
