You will manage high-acuity cases and support families in their homes. The position focuses on quality, not volume, with responsibilities across home visits, care coordination, and ongoing patient support.

Qualifications

• Licensed as a Nurse Practitioner in the practicing state.
• Federal DEA license/eligibility.
• CPR/BLS certification.
• Board certification in the field.

Responsibilities

• Provide in-home primary care to high-acuity patients.

Responsibilities

The Nurse Practitioners we seek are those who have directly related experience in home health, geriatrics, hospice and primary care. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.

Qualifications

• Licensed as a NP in the State where HarmonyCares Medical Group office located
• Federal DEA License/eligibility
• Geriatric training/experience preferred
• Active CPR/BLS Certification
• Board Certification
• Valid and unencumbered driver's license with good driving record
