We are seeking an Advance Practice Provider to manage chronic conditions of a high-risk geriatric patient panel, Monday through Friday,

Qualifications

• Licensed NP or PA with state licensure in the practice location.
• Federal DEA license eligibility.
• Geriatrics and home health experience preferred.
• Active CPR/BLS certification and board certification.
• Must maintain a valid driver's license and good driving record.

Responsibilities

• Provide in-home primary care to high-acuity geriatric patients Monday through Friday.
• Focus on patient-centered care, taking time to address patient needs and preferences.
• Collaborate with an interdisciplinary team to coordinate care.

Responsibilities

The Advance Practice Providers we seek are those who have directly related experience in home health, geriatrics, hospice and primary care. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.

Qualifications

• Licensed as a NP or PA in the State where HarmonyCares Medical Group office located
