Job summary

HarmonyCares is seeking a Value-based Advance Practice Provider (NP/PA) to manage high-risk geriatric patients in home health across Michigan. Mon–Fri, no weekends or evenings, start around 8:00 AM, finish by 4:30–5:00 PM.

Use Athena EMR plus Marvix AI Scribe to complete charting during the workday. In our physician-led, team-based model, you will deliver home-based primary care with competitive compensation and a comprehensive benefits package in select markets.

Qualifications

• Licensed NP or Physician Assistant in Michigan.
• DEA license/eligibility required.
• Geriatrics training/experience preferred.
• CPR/BLS certification.
• Board certification is preferred.
• Valid driver's license with clean record.

Responsibilities

• Provide home-based primary care to high-acuity geriatric patients.
• Deliver patient-centered care focused on needs over volume.
• Collaborate in a physician-led, team-based model using EMR (Athena) and AI scribe (Marvix).

The Advance Practice Providers we seek are those who have directly related experience in home health, geriatrics, hospice and primary care. The APPs who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.

Qualifications

• Licensed as a NP or PA in the State where HarmonyCares Medical Group office located
• Federal DEA License/eligibility
• Geriatric training/experience preferred
• Active CPR/BLS Certification
• Board Certification
• Valid and unencumbered driver's license with good driving record
