Job summary

HarmonyCares seeks a Nurse Practitioner to provide in-home primary care for high-acuity patients, focusing on patient-centered assessments and care planning.

The NP will work with a multidisciplinary team to coordinate services, travel to patient homes, and deliver comprehensive care in a home setting.

Qualifications

• Licensed NP in the state of practice
• Federal DEA license or eligibility
• Geriatric training/experience preferred
• CPR/BLS certification
• Board certification in NP specialty
• Must maintain a valid driver’s license and good driving record

Responsibilities

Responsibilities

The Nurse Practitioners we seek are those who have directly related experience in home health, geriatrics, hospice and primary care. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.
