Job summary

Jewish Family & Children's Service of Arizona is seeking a Health Navigator to coordinate primary care and behavioral health services for clients in Phoenix. You will work with healthcare teams to promote whole-person care, address barriers to access, and conduct SDOH screenings with referrals to resources as needed.

The role supports preventive care, chronic disease management, and collaboration across service lines to close care gaps.

Qualifications

• Bachelor’s/associate degree with health care or behavioral health exposure preferred.
• Strong communication and written skills required.
• Must have background check clearance and clean driving record.

Responsibilities

• Coordinate one-on-one primary care and behavioral health services for clients.
• Connect with providers to promote whole person care and reduce barriers to access.
• Conduct SDOH screenings and refer to community resources as needed.
• Support preventive care visits and chronic disease management.
• Collaborate with care teams to close gap in care and update plans.
• Coordinate transportation and assist with non-clinical client advocacy.
