Job summary

InclusivCare in Arizona is seeking a Behavioral Health Navigator to provide administrative support for the Behavioral Health Provider team, school-based services, and patients. You will input data into the EMR, coordinate appointments, transportation, and medication requests, and support efforts to close gaps in care.

Qualifications

• Bachelor's degree in social sciences or related field; or 3+ years of relevant experience.
• Experience in outpatient clinics with administrative support to health leadership preferred.
• Familiarity with behavioral health concepts and evidence-based practices.

Responsibilities

• Manage referral caseload and schedule queue for Behavioral Health appointments.
• Support patients in person and by phone to access services, coordinating transport and meds.
• Coordinate continuity between Behavioral Health and Primary Care Providers.
• Provide psycho-educational information to students and staff.
• Address no-shows and cancellations to reduce scheduling gaps.
• Analyze scheduling patterns using reports and optimize allocations.
• Participate in team meetings and quality improvement initiatives.
• Attend meetings to report progress in the behavioral health dept.
• Assist Community Health Worker with outreach efforts.
• Maintain confidentiality and HIPAA compliance.
• All other duties as assigned.
