Job summary

Circle the City is seeking a Patient Navigator to support the Integrated Health Care team in Phoenix, addressing social determinants of health and improving access to care. You will assist patients with scheduling, transportation, housing, and referrals, coordinating with community partners to connect services.

This frontline public health role requires strong communication, critical thinking, and the ability to build trusting relationships across diverse populations, with responsibilities

Qualifications

• Must be 21 years of age or older.
• High school diploma or GED required.
• Spanish speaking preferred.
• Strong interpersonal and communication skills.
• Experience with culturally diverse, homeless populations advantageous.
• Demonstrates critical thinking and problem-solving abilities.
• Valid Arizona driver's license required.
• Arizona DPS Clearance Card required.
• Basic Life Support (BLS) certification.

Responsibilities

• Refer and connect patients to unmet social determinants of health services.
• Assess DES benefit eligibility and coordinate enrollment for uninsured patients.
• Explain care plans; document activities and progress in a timely manner.
• Provide education and support for patients with chronic health concerns.
• Coordinate individualized care and patient navigation with the care team.
• Collaborate to remove barriers to efficient clinic delivery and services.
• Participate in team meetings and quality improvement projects.
• Maintain positive relationships with patients, providers, staff and community partners.
• Engage in community outreach and represent the agency in outreach activities.
• Complete required documentation in EMR within set timeframes.
