Job summary

Johns Hopkins Home Care Services in Baltimore is seeking an experienced registered nurse to provide in-home care, coordinate services, and educate patients and families in their own homes.

The role supports a flexible schedule within assigned territories (Baltimore County, Baltimore City, Harford, Howard, and Anne Arundel). Qualified candidates have nursing licensure in MD, BLS/CPR, and 2+ years in acute care or home care settings.

Qualifications

• Associates in Nursing required; BSN preferred.
• Current MD RN licensure or eligibility.
• Active American Heart Association or Red Cross BLS CPR.
• Valid Maryland driver’s license and auto insurance.
• At least 2 years of Acute Care, ICU or ED experience.
• 2 years of home care experience preferred.
• Passion for educating patients in their home environment a MUST.
• Experience with electronic documentation (i.e. Epic).

Responsibilities

• Provide in-home nursing care and develop plans of care for patients.
• Teach patients and caregivers about health management.
• Coordinate referrals and services with the agency and community.
• Advocate for patients to achieve health goals.
• Schedule visits and ensure adequate service provision.

Job description

Provides, coordinates, teaches, AND supervises nursing care for a group of patients in the setting of the patients' home. Employs the nursing process to develop and implement plans of care for patients requiring in-home health and support services. In addition to specific and skilled treatments and procedures, the nurse provides health teaching to patients and care givers, preventive health screening, referral and coordination of services in the agency and the community, and advocacy for the patient in obtaining health-related goals. The Case Manager ensures adequate service provision, including scheduling of visits, based on patients identified needs and priorities for care. Complies with all Home Health Services policies and procedures.

Work Location

Baltimore County, Baltimore City, Harford County, Howard County, and/or Anne Arundel County (you will be assigned to specific territory)

Work Hours

Must be available from 8:30am-5pm. However, patient visit times are flexible.

Required Qualifications

• Associates in Nursing required; BSN preferred
• Current RN licensure in MD or willingness to obtain needed license.

Salary

Minimum $39.64/hour - Maximum $61.45/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
